Cancellation & Reservation Policies

We know policies aren’t the most exciting read. But knowing them means you’ll never be caught off-guard by a fee or a lost credit. Everything you need is on this page.

1. Class Cancellation

Membership Clients

Early Cancellation (12+ hours before class):

Log into your FORM50 account and select “cancel reservation” with no penalty.

Late Cancellation (under 12 hours before class):

No-Show, No Cancellation:

Only medical conditions with a doctor’s note will be excused from our cancellation policy.

Credits & Package Clients

Early Cancellation (12+ hours before class):

Cancel with no penalty. Your credit returns to your package.

Late Cancellation (under 12 hours before class):

You lose the class credit.

No-Show, No Cancellation:

You lose the class credit and receive a $25 fee.

Guest Cancellations:

Only medical conditions with a doctor’s note will be excused from our cancellation policy.

2. How to Cancel a Reservation

  1. Log into your FORM50 account at form50fitness.com/my-account or open the FORM50 app.
  2. Go to your upcoming reservations.
  3. Select “Cancel Reservation” next to the class you want to cancel.
  4. Confirm the cancellation. You will receive a confirmation email.

Cancellations must be completed at least 12 hours before class to avoid a penalty.

You may join one waitlist spot per class per day. Signing up on the waitlist is like signing up for class with the intention that you are going to make it.

Unused Credits

Credits from 4x or 8x per month memberships do not roll over. Any unused credits expire at the end of each billing cycle.

7. ClassPass & Gympass

All ClassPass and Gympass reservations are managed through third-party systems. We cannot alter or cancel your reservation on our end. Contact the customer service of the app you booked through directly.
